Morteza Hosseini, aged 28, was last seen in Merrylands about 9am on Thursday, September 12.
Cumberland Police have serious concerns for his welfare as he suffers from a number of health conditions and requires regular medication.
Morteza is described as being of Middle Eastern/Mediterranean appearance, 170cm tall, medium build, with short black hair.
He was last seen wearing a grey hooded top, black pants and blue sandals.
He is known to frequent public parks in the Bankstown area.
Police are urging anyone with information on Morteza’s whereabouts to contact Crime Stoppers 1800 333 000.
